Abstract:  Improving the quality of health care received is a priority for us all. However, the healthcare system in the U.S. is not a typical service industry. Globally, the U.S. has the highest spend per capita but lower proxy health outcomes compared to other first world countries. As Paul Batalden’s quote goes “Every system is perfectly designed to get the results it gets.” Because of the healthcare system in the U.S., typical QI methodologies are difficult to apply (PDSA, 6 Sigma, Lean, Agile, etc.) and get meaningful improvement in the health care received. A more effective approach requires incorporation of population health concepts, public health concepts and the Social Determinants of Health. This approach has been termed “upstream QI.” It follows the Pareto principle. Through Deming’s Lens of Profound Knowledge, the current heathcare/ health care system will be discussed. A unique approach will them be proposed.

About Robert Flora:  Dr. Flora is currently the Chief Academic Officer and Vice President of Academic Affairs at McLaren Health Care. He assumed this role on October 17, 2016. He previously served as Director of Medical Education and Designated Institutional Officer for Providence Hospital and Medical Center in Southfield, Michigan. He is currently a Professor in the Department of Obstetrics, Gynecology, and Reproductive Medicine at Michigan State University College of Human Medicine and a Clinical Professor at the Michigan State University College of Osteopathic Medicine. He attended undergraduate school at the University of Michigan obtaining a Bachelor of Science degree with studies in biophysics and engineering science.  Medical school and Ob/Gyn residency were completed at the Medical College of Ohio, and then he went into private practice for three years in Toledo. Returning to academics, a fellowship in Urogynecology and Reconstructive Pelvic surgery was completed at Mt. Sinai /University of Connecticut and Albert Einstein College of Medicine/Montefiore Medical Center. In 2003, Dr. Flora received his Master of Business Administration degree from the University of Tennessee in Knoxville. He completed a Master of Public Health program through the University of Akron /NEOMED and graduated in August 2008.

Dr. Flora is board certified in Obstetrics and Gynecology and Urogynecology and Reconstructive Pelvic Surgery (American Board of Obstetrics and Gynecology, Inc.), as a Certified Physician Executive (CPE) (Certifying Commission in Medical Management), Certified Professional in Patient Safety - CPPS (IHI) and Certified Professional in Healthcare Quality – CPHQ (NAHQ), He is a fellow of the American College of Health Care Executives. He is a current member of ASQ.
