Our Organization defines Audit Findings (Non-conformities) as Major, Minor, Observation and Opportunities for Improvement (OFI). It has been doing this for long. I am trying to find out guidelines or documents that speak about this. I find a small reference in ISO 19011:2018, Section 6.4.8 - "Nonconformities can be graded depending on the context of the organization and its risks. This grading can be quantitative (e.g. 1 to 5) and qualitative (e.g. minor, major)."
We need to define when a finding should be Major or Minor or Observation or OFI.
Another question that arises is - "Is Root Cause / Corrective Action required for OFI?"
