CAPA Systems That Actually Work: Investigations, Root Cause Analysis, and Preventing Recurrence was presented by Mathangi “Maddy” Srinivasan, Quality Assurance Manager for Class III cardiovascular medical devices, to the ASQ Temecula Valley Section. The session explored why many CAPA programs successfully close records yet fail to eliminate recurring problems. Attendees were guided through a practical framework for building effective investigations, developing evidence-based problem statements, collecting meaningful data, selecting the appropriate root cause analysis tools, and conducting effectiveness verification that demonstrates real improvement. The presentation emphasized that root cause is typically a system condition rather than an individual error and that sustainable corrective actions depend on disciplined investigation rather than assumptions or retraining alone.
The discussion also highlighted the impact of FDA’s Quality Management System Regulation (QMSR), which became effective in Feb 2026 and places greater emphasis on risk-based thinking, management oversight, and integration between CAPA, risk management, design controls, supplier quality, and post-market surveillance. Through a combination of current regulatory trends, FDA enforcement data, real-world warning letter examples, and industry-based case studies, attendees saw how premature conclusions, inadequate effectiveness checks, and disconnected quality systems often lead to recurring failures. The session concluded with practical guidance for advancing CAPA maturity, focusing on recurrence prevention, meaningful metrics, and creating a culture that prioritizes solving problems rather than simply closing investigations. by Mathangi “Maddy” Srinivasan August 19, 2026
