The Healthcare Division is soliciting nominations for the 2023 Service Award
Eligibility requirements include:
▪ Nominations will be accepted from Healthcare Division members only
▪ Candidates are required to be a member of the Healthcare Division
▪ Self-nominations will not be accepted
▪ Multiple service awards may be awarded in the same year
▪ Recipients may be recognized only once every 10 years. Each recognition must reflect a totally new contribution.

▪ A complete list of requirements, along with the application form, can be
found in the attached document below
▪ Nominations and support documentation must be sent via email to
Danny Duhan, HCD Awards Chair, at dduhan@memberleader.asq.org
▪ ASQ and HCD assumes no responsibility for applications and support
documentation not received by the September 30, 2023
