Deepwater Horizon - What Went Wrong?

Short Description:
On April 20, 2010, an explosion and fire occurred on the Deepwater Horizon semi-submersible mobile offshore drilling unit, which was owned and operated by Transocean and drilling for BP in the Macondo Prospect oil field about 40 miles southeast off the Louisiana coast. The explosion and subsequent fire resulted in the sinking of the Deepwater Horizon and the deaths of 11 workers; 17 others were injured. The same blowout that caused the explosion also caused an oil well fire and a massive offshore oil spill in the Gulf of Mexico, considered the largest accidental marine oil spill in the world, and the largest environmental disaster in United States history
The loss of life and the subsequent pollution of the Gulf of Mexico were the result of poor risk management, last-minute changes to plans, failure to observe and respond to critical indicators, inadequate well control response, and insufficient emergency bridge response training by companies and individuals responsible for drilling at the Macondo well and for the operation of the drilling platform.

Jeff will led the discussion using publicly available information. This will be an interactive discussion, with quality experts asked to give their opinions on the problems and how this tragedy could have been prevented.
Speaker Background:
Jeffrey Parnes is currently retired. His product assurance career started 50 years ago when he became a Quality Assurance Trainee for the Defense Contract Administration Services (DCAS).
- As a Defense Logistics Agency (DLA) employee, his responsibilities included wood products, computer software, statistics and Defense Supply Center operations.
- In the private sector, Jeff has worked for various firms in the Washington Metropolitan area, including:
- Computer Sciences Corporation as a computer scientist working as a sub for Boeing supporting the Peace Shield Project
- Hughes Training Corporation as a software quality manager and configuration manager on a variety of trainers (V22, LCAC, T45, & RAF F-16)
- Advanced Communications Systems as support for Space and Naval Warfare Systems Command (SPAWAR)
- Orion Scientific supporting the Defense Intelligence Agency (DIA)
- Planning Research Corporation/Northrop Grumman supporting multiple computer and hardware system deployments
- Electronic Data Systems supporting the Federal Reserve
- Unisys supporting multiple computer system deployments
- ITT as a product assurance manager supporting the FAA's Automatic Dependent Surveillance – Broadcast (ADSB) systems deployment
- Maximus supporting nationwide health care systems
- Various contractors (ASM Research, Accenture Federal Systems, CSRA, and others supporting Department of Veterans Affairs system deployments and upgrades
Jeff has been a member of ASQ Section 0511 since its formation in 1983, having joined ASQC in 1979. He has served multiple terms as the section's Treasurer and Chair, and has maintained the ASQ0511 website since before 2007. He is also a board member of the Configuration Management Working Group of Metropolitan Washington.
This will be Jeff's third presentation to the membership, previously presenting in pandemic times and last month.
