Nominations for the Six Sigma Forum Award are now being accepted through August 29, 2025.

The Six Sigma Forum Award for the Advancement of Six Sigma recognizes outstanding achievements, efforts and contributions from quality professionals that further the Body of Knowledge and practice of the Six Sigma methodology. Given at the Lean and Six Sigma Conference, this award is presented to individuals who have expanded the use of Six Sigma to drive breakthrough improvements. The next LSS conference is February 22-24, 2026 in Phoenix, Arizona.
To nominate someone for the award, please submit the following documents* which demonstrates the nominee has clearly met or exceeded the defined criteria:
- Completed the Nomination Form
- Two letters of recommendation
*Please note the Six Sigma Forum Award Committee may also request additional documentation, if required. Additionally, Self-nomination is not permitted, and previous winners are not eligible.
Nominations are due by August 29, 2025. Please e-mail the nomination form and the 2 letters of recommendation to bpeck@asq.org.
