Summary: A systemic root cause is the underlying issue within a system that leads to recurring problems or failures. Unlike surface-level causes, which might only address symptoms, a systemic root cause is deeply embedded in the structure, policies, processes, or culture of an organization, industry, or society. Use the “5 Whys” Technique – Keep asking “Why?” until you reach the core issue.  Apply Systems Thinking – Look at how different factors interact within the system. Analyze Patterns Over Time – Spot trends rather than isolated incidents. Engage Multiple Stakeholders – Get insights from different perspectives within the system. Addressing systemic root causes leads to long-term solutions, rather than just treating symptoms.
