Healthcare Division - Soliciting nominations for 2025 Testimonial Awards

The Healthcare Division (HCD) is soliciting nominations for 2025 Testimonials Awards to
HCD members who have served the Division and the Society with distinction for an
extended period.

For an individual to be considered, the following is required:

• The candidate’s name, address and contact information (including phone and email)
• A summary of the candidate’s ASQ Activities (memberships, positions, service, etc.)
• Previous honors the candidate has received (ASQ and others)
• A description on why the candidate should receive this award
• Testimonial wording (what you want to appear on the certificate)
• Five endorsements

ASQ and HCD assumes no responsibility for applications and support documentation not received by the October 10, 2025 deadline.
