Thorsten is currently the Director of Quality at PwrQ, He has been in quality leadership positions spanning more than 20 years in several industries, including steel, fine powder chemicals including excipient products, and biologics pharmaceuticals. He is an accomplished Quality Management, Continuous Improvement & Operational Excellence Leader with proven vision towards developing strong and sustainable strategies and tactics to build individuals and teams towards collaborative data-driven business solutions via continuous quality and project management methodologies to achieve significant tangible improvements in quality and cost metrics, using a robust foundation in root cause analysis methodologies such as Six Sigma, Apollo, and TapRoot®. He is certified six sigma black belt, was the program manager for the lean sigma initiative at WR Grace, and holds CMQ/OE and CQE ASQ certifications, as well as PMI project management certifications, and am a certified ISO 9001:2015 Lead Auditor.
The program topic for the evening is "Human Factors in Root Cause Analysis."
So you are working on yet another human error quality event. Are you using the best approach and actually digging into the components that affect human behavior. In this presentation we will review root cause analysis, specifically the human factor components. An overview of the five principles of human behavior in root cause analysis will be provided. The objective is that the audience is left with a better understanding of factors involved in human performance excellence and be able to apply the principles to actual events in their workplace or home life.
The tutorial will be: Using a checklist to perform human factors investigation.
The group will be provided a failure scenario and be split into teams to delve into different components of the event and derive meaningful action plans.
