    Greetings,
    My name is Thomas J. Lalka and I am a student at the University of Phoenix working on a Doctorate in Business Administration. I am conducting a research study The Purpose and Future of Adopting a Quality Model: An Appreciative Inquiry.
    The purpose of the research study is to get expert opinion on what is the purpose of adopting a quality model and what do quality experts see as the future of quality modeling. The information will be collected in a survey. The survey is instrumental to my dissertation research, and I would greatly appreciate assistance in completing the survey on quality models.
    The study is looking for respondents that are quality experts to answer questions on the purpose of adopting a quality models and the future of quality modeling. This is an invitation for ASQ members working in the area of quality models to participant by giving useful information to other quality model experts. Participant names will not be collected by SurveyMonkey; participation will be anonymous.
    To complete the online survey, please click on this link or copy the link and place it in a browser: https://www.surveymonkey.com/r/5GBFHMQ Clicking on the link will lead participants to an online informed consent process. Participants will not be able to participate in the survey without first providing consent.
    Participation in the survey is voluntary and participants may discontinue taking the survey at any time. Participation involves completing the online survey, which will take approximately 10 to 20 minutes to complete. Your participation will involve completing the survey within seven days.
    Participant’s name will not be collected, but the location and type of organization is requested in the survey to ensure that the organization is located in the United States. In this research, there are no foreseeable risks to the participants. There is no compensation to participants for responding to the survey. To ensure that all information will remain confidential, participants do not include their name. If you choose to participate in this research study, please answer all questions as honestly as possible. Participation is strictly voluntary, and participants my elect to omit answers or discontinue the survey at any time.
