About Topic


Since the early 1990s, the Swiss Cheese Model (SCM) has been used in the fields of accident prevention and environmental incident investigation. By extension, this model can be applied by quality practitioners to improve risk mitigation strategies to prevent and understand quality failures in manufacturing and service environments. The development of the model, it’s underlying principles and its application will be discussed. A health care example and a quality management example will be presented to illustrate the utility of this model in our current context.



About Ralph


Ralph Krueger has been employed in Quality Management activities for over 30 years for a large steel manufacturing facility in Hamilton, Ontario. He is an experienced lead auditor for a variety of international standards including ISO 9001, ISO 17025, ISO 14001, ISO 45001 and ISO 50001. As a Senior Member of the Hamilton section of the ASQ, he has held the positions of Secretary and Program Coordinator.
